HOSPITAL TRANSITION PATHWAYS

Informed by best practice and driven by a passion to improve service provision and experiences of care.
ConNetica has dedicated time, funds and resources to improving mental health related hospital transition pathways. ConNetica Directors, Adjunct Professor John Mendoza
(former) & Marion Wands attended a hospital transition flow conference in Boston and visited leading USA Emerging Departments and Dispatch Centres.

Reflection upon our lived experience, combined with our research, conference learnings and site visits  has resulted in ConNetica’s comprehensive integrated Mental Health Hospital Transition Pathway framework and methodology. This has been used with 10 hospitals across Australia.

ConNetica has and continues to undertake detailed literature reviews of relevant research to ensure our robust framework is reflective of best practice. We support our clients to understand  “the potential” for improvements and actions to attain desired service enhancements.
