(former) & Marion Wands attended a hospital transition flow conference in Boston and visited leading USA Emerging Departments and Dispatch Centres.
Reflection upon our lived experience, combined with our research, conference learnings and site visits has resulted in ConNetica’s comprehensive integrated Mental Health Hospital Transition Pathway framework and methodology. This has been used with 10 hospitals across Australia.
Best practice information
ConNetica has and continues to undertake detailed literature reviews of relevant research to ensure our robust framework is reflective of best practice. We support our clients to understand “the potential” for improvements and actions to attain desired service enhancements.

ConNetica has completed 10 Hospital Transition Pathway Service Frameworks with:

ConNetica's Hospital Transition Pathways
The elements of data collection and analysis include mapping of existing transition pathways into, through and out of hospital and documentation of roles, responsibilities and associated competencies. Key steps:
- consumers and family/carers identify their needs in relation to information, services and supports
- current and “to be” pathways are identified
- all stakeholders identify barriers & opportunities for improved service, collaboration, KPIs
- new processes are implemented
